| The Initial Steps to Working With Us |

| 1. |
Call our office toll-free at 1.888.466.3990 to request for a Pre-employment Telephone Screening Session with our Service Coordinator Assistant. |

Depending on the outcome of your Pre-employment Telephone Screening Session, you could be given the go-signal to download, complete and print the following Employment Screening forms to be submitted to our office on the day of your interview. |

Upon completion of the above forms, call us again to schedule an appointment for an interview with our Service Coordinator. You must bring and show at least two (2) original forms of US government-issued ID's during your interview. |

After the interview, reference and background checks shall be conducted. |
| 5. |
Within a week from the day of your interview and submission of completed Employment Screening forms, you will receive a call or letter from us regarding the result of your Employment Application process. You could then be a part of our Roster for Pre-Qualified Caregivers. |
